Appointments & Fees
Make an appointment: To make an appointment, please call (608) 556-2388 or email me at firstname.lastname@example.org. I will contact you as soon as possible.
Fees: My fee is $150 for a 50-minute session.
Payment: Payment is due at the time of service. I accept cash, checks and credit cards.
Insurance: I accept the following insurance:
- The Alliance
- Concern EAP
- Lyra Health EAP
- United Behavioral Health
Please check with your insurance provider prior to your first visit to make sure you understand your eligibility for out-of-network coverage. At your request, I will provide you with a detailed receipt that you may submit directly to your insurance provider.
Please provide at least 24 hours’ notice if you cannot keep an appointment. Appointments missed or canceled without 24 hours’ notice are subject to the full session fee. You may notify me of a cancellation by phone at (608) 556-2388.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least one (1) business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
